U.S. Immigration Physical Examinations - Instructions and FAQ's

The following is a list of items you are required to bring to your examination. Following that are several Frequently Asked Questions that may be helpful to you in preparing for your immigration physical.                  

  1. INS Form I-693, pre-completed with your information. This form is available online by clicking here.
  2. Your personal photo identification with a photo
  3. Your passport
  4. Hospital or clinic records
  5. Prior chest x-rays and any other diagnostic tests
  6. Vaccination status verification
